Health Information
When calling in to report an illness, please be SPECIFIC regarding your child’s symptoms.
Students must be fever free (without fever reducing medication) for 24 hours before returning to school.
Please notify the nurse if your child is diagnosed with Strep throat or head lice.
Allergies
Due to the life threatening nature of peanut/nut allergies, no foods with peanuts or nuts, nut products, or peanut/nut warnings are allowed in the classroom. No home baked, or baked items from a store or bakery, are permitted in the classroom due to the risk of cross contamination of foods baked with peanuts or nuts. Foods with peanuts, nuts, or nut products are allowed in the cafeteria for lunch because a peanut/nut free table is provided for children with these allergies.
Medication At School
A medication administration form must be completed by the physician and signed by the parent for all students who have medication at school. This includes any over the counter medications a student may need.
Health Examination Requirements
The State of Illinois requires all students entering kindergarten have a vision exam and physical exam done on, or after, 9/01/10. The immunization record must also be up to date. In addition, all students entering kindergarten and second grade are required to have a completed dental exam on file by May, 2011. You may return the required forms whenever they are completed. Student physical and vision forms MUST be turned in by October 15. Per State law, students who have not provided a current physical and immunization record by that date will be excluded from school.
The State of Illinois requires that all transfer students from out of state present proof of a current physical exam completed on a State of Illinois Child Health Examination Form within one year before entering school. This form is available from your health provider’s office, or from the school nurse. A Dental and Vision exam is also required. Forms are available on the district website or from the school nurse.
Lake Forest Hospital offers free immunizations through CareCoach , a mobile immunization and health screening clinic, which visits communities throughout Lake County each month. No appointment is necessary. Please call 847-535-6709 for schedule information.
